Group Disability Insurance
Every 90 seconds someone files for bankruptcy in the wake of a serious illness (The American Journal of Medicine, Vol. 122, No. 8, August 2009)
Group Disability Insurance protects you from loss of income due to an unexpected illness or injury. This program is an inexpensive way to protect your income, yourself and your family.
New employees may enroll in Group Disability Insurance guaranteed issue (no medical exam required) within their first 90 days of employment or during open enrollment periods. This plan is available to state university system employees through payroll deduction.
Disability insurance replaces two-thirds of your income tax-free to a maximum of $15,000 per month* payable until Social Security Normal Retirement Age (SSNRA**). Benefits are paid after either 30 or 90 days, according to the elimination or waiting period you choose.
In the event of a catastrophic disability-one that causes the loss of two of six Activities of Daily Living or a Severe Cognitive Impairment-an Assisted Living Benefit pays 80% of your income to a maximum of $18,000 until Social Security Normal Retirement Age. Also in the event of catastrophic disability, a Lifetime Benefit pays two-thirds of your predisability income to a maximum of $15,000 for life starting at Social Security Normal Retirement Age. Catastrophic Disability benefits pay after 90 days of disability, and the loss must occur while you are eligible for disability benefits.
The premium for the 90-day Plan (Long Term Disability Insurance) is $0.59 per $100 of monthly income. With the 30-day Plan (Short Term Disability and Long Term Disability Insurance), for an additional $0.26 per $100 of monthly income, you can begin receiving benefits a full 60 days earlier in the event of a disability ($0.59 per $100 of Long Term Disability coverage plus $0.26 per $100 of Short Term Disability coverage).
* If you elect the 30 day plan, benefits from day 30 to day 90 will be paid on a weekly basis to a maximum of $3,462 under a Short Term Disability policy. After 90 days of disability, benefits are paid monthly through a Long Term Disability plan as described above.
** Social Security Normal Retirement Age means your normal retirement age under the Federal Social Security Act, as amended. Please reference the following maximum benefit schedule for participants over age 62.

To apply, please complete the Enrollment Form below and turn it in to your Human Resources department.
Be sure to indicate if you are a "New Applicant" without current coverage, or if you are making changes to your current coverage.

Non-Open Enrollment Applicants
Employees applying for coverage outside of the 90 day* new hire or guaranteed issue period must complete an Evidence of Insurability (EOI) application along with the enrollment form. EOI applications should be sent directly to The Gabor Agency for processing with The Standard.
Gabor will notify you and your employer if your application is approved. To apply please contact your local Gabor representative.
* University of Florida employees have 60 days to enroll as a new employee.
